    LPN – Foot Care - part-time, temporary

    Wetaskiwin Primary Care Network (“PCN”)

    Reports to: Clinical Manager

    Job Description

    Under the supervision of the Clinical Manager Wetaskiwin Primary Care Network, the LPN - Foot Care will provide comprehensive foot care services to the patient population from within the Network. The LPN will work to full scope of practice with an emphasis on providing appropriate foot care services and documentation, while working collaboratively with the physicians to develop, implement and evaluate care plans. This position provides care directly related to diabetic foot care and assessment of same, as well as determine and carry out advanced foot care nursing interventions.

    Key Responsibilities
    · Work collaboratively within a team environment to enhance the delivery of primary care services

    · Develop therapeutic relationships with patients to provide health services based on highly developed nursing knowledge and skills

    · According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate follow-up

    · Make professionally autonomous decisions in managing patients and offering a complementary source of care to that offered by medical practitioners

    · Assist in the implementation of strategies for improving the quality and provision of care for the practice population

    · Initiate and maintain community and regional partnerships

    · Link PCN initiatives to the AHS programs and services

    · Track, monitor and report on encounters and identified performance indicators


    Clinical Role

    · Work in conjunction with the physicians or independently as appropriate, to assess the needs of patients who are referred for foot care

    · Assist in the delivery of disease management, health promotion programs, and direct clinical care/ treatment, according to practice policy and protocols

    · Provide chronic disease management, geriatric care, mental health screening/referral, as well as monitoring progress, response to treatment, and promoting the preventive aspects of patient care

    · Initiate appropriate investigations


    Qualifications
    · Licensed Practical Nurse – current registration

    · Current CPR

    · Foot care training – certificate of completion


    Experience
    · Preference given to those with at least 3 years nursing experience

    Knowledge and Skills
    · Clinical skills in advanced foot care

    · Knowledge of community based resources

    · Ability to build a good working relationship with healthcare professionals, clients and other members of the community

    · Respect and understanding of confidentiality

    · Knowledge of and comfort with computer applications such as Microsoft Word, Outlook, NetCare, electronic medical records, etc.

    · Skills in negotiation and advocacy

    · Skills in dealing effectively with a variety of internal and external contacts, including physicians, coworkers, community agency representatives, patients and families

    · Ability to develop new skills and apply theory to practice

    · Flexibility in approach to patient and interpersonal situations

    · Ability to make evidence-based decisions

    · Ability to monitor and maintain clinical practice standards, and knowledge of relevant Acts and legislation
